Birthday Party Agreement Form

To confirm your reservation, please have a parent or guardian initial this form and submit.

Party Requirements

  1. $25 deposit is required at time of booking mail check to Heaven Hill Farm 451 State Route 94 Vernon, NJ 07462
  2. With the exception of inclement weather, a non-refundable $25 cancelation fee will be charged for any party that is cancelled within 24 hours of scheduled time.
  3. Payment must be made prior to the time of party.
  4. Parents must show up 30 minutes prior to scheduled party time to be prepared for when your guests arrive.
  5. Adult supervision is required and you are responsible for getting the children to all of the activities. Any parents accompany the party must pay the admission fee if going in to any of our events.
  6. A Heaven Hill Farm employee will greet you at check-in, help you get set up, however we do not provide you with a party attendant. It is up to you to supervise and get your guests to all the activities included.
  7. Remember to dress the children accordingly.
